Annetta Troche?

Who is

Annetta Troche grew up in South India where her father worked as a missionary doctor for over thirteen years. From a young age she followed along beside her father learning from him during rounds in the wards, office consultations and surgery in the operating room.

After moving to the United States, Annetta studied nursing in New York and obtained her RN/BSN in 1991.  Over the next two decades she worked in many areas of nursing including community health, medicine, surgery, oncology, cardiology and acute care before returning to school for her advanced practice registered nurse degree.

Annetta obtained her masters in nursing science and her nurse practitioner degree from the University of Connecticut in 2012.   Her studies focused on the diagnosis and treatment of acute and chronic medical conditions in adult, geriatric and acutely ill patients. She performed her graduate clinical rotations with the leading physicians at The Hartford, John Dempsey and Manchester Hospitals and is now certified in acute care, adult-geriatric care and wound care.

Annetta has worked with older adults in nursing homes, rehab centers and their homes to help manage acute and chronic conditions in order to reduce their need for hospitalizations.

GENERAL

HOW DO I REACH ANNETTA IN THE CASE OF AN EMERGENCY?

 

If you have chest pain, shortness of breath, head trauma, loss of consciousness, deep laceration or a possible fracture, call 911 and go to the ER unless you have decided you do not wish to be hospitalized for any reason and have specified these wishes in your advanced directive. Please keep in mind that Annetta may not be available immediately, so please make sure that you are taken care of in emergent situations by calling 911.

 

WHAT KIND OF ISSUES CAN ANNETTA TREAT?

 

Annetta is able to treat most acute and chronic conditions in older adults, such as conjunctivitis, sinusitis, ear infections, allergies, rash, laryngitis, bronchitis, pneumonia, flu, reflux, gastroenteritis, high blood pressure, heart failure, diabetes, COPD, gout, insomnia, urinary tract infections and more. Blood tests and minor procedures are done right in your home.

HOW DO I SCHEDULE AN APPOINTMENT?

DO I NEED A DOCTOR'S ORDER TO SCHEDULE AN APPOINTMENT?

 

No, any patient, family member or healthcare provider can make a referral for a visit. Annetta is an independent medical provider.  If you want care coordinated between your regular doctor and Annetta, you need to sign a form giving your provider permission to release information to Campbell Medical Care LLC, and she will be happy to share visit notes with your doctor. 

WHAT DAYS WILL ANNETTA SEE PATIENTS?

 

If possible, try to make calls between 8 am and 5 pm Monday through Friday. Weekend and evening visits can be arranged occasionally if necessary.

HOW FREQUENTLY WILL ANNETTA VISIT ME?

 

Routine visits are every 6 months

 

HOW FAR IN ADVANCE DO I NEED TO SCHEDULE AN APPOINTMENT?

 

Please try to schedule an appointment at least 5 days in advance, but if there is an acute condition it is likely Annetta will be able to come within a 24 hour period.

IN WHAT AREA WILL ANNETTA SEE PATIENTS?

 

Annetta will see patients within a 15 mile radius of Windsor, Connecticut.

APPOINTMENTS

 

WHAT HAPPENS DURING A TYPICAL IN-HOME APPOINTMENT?

A visit from Annetta is just like a regular visit to your health care provider but in the comfort of your own home. On the first visit, Annetta will go through your medical history and give you a physical as well as determine if there are any medical tests that need to be done, as well as help you sign the necessary release forms, get the right insurance information, and fill out a medical questionnaire. The first visit may take up to one hour. Follow-up visits tend to focus on any medical problems or concerns that have come up since the previous visit, and so should only average 15 to 30 minutes.

 

SHOULD MY FAMILY BE PRESENT DURING APPOINTMENTS?

 

If you have a POA, they will have to supply a copy of their POA/COP form. Family members can be very helpful in answering questions if you tend to forget things, and it is greatly appreciated if family members can be present during the first visit. The more accurate and thorough the information received, the better Annetta can care for you. If your family is not able to be present, Annetta may try to get information over the phone prior to the visit. It is not as important for family to attend follow-up visits, but they are still welcome!

DO WE ACCEPT INSURANCE?

We do NOT accept insurance, but we CAN provide a receipt that can be submitted to your insurance company for reimbursement. Understanding that many people are struggling financially now, the cost of in-person visits has been reduced to $95 and phone consultations to $15.  Prescription renewals are free of charge. 

 

HOW DO I PAY FOR APPOINTMENTS?

 

By cash, check or credit card during or immediately after the appointment. Please make checks out to "Campbell Medical Care, LLC".
